Health Care Pricing

How are medical prices (charges) determined?  Mainly by the market power of the two negotiating players (health insurers and health providers), not necessarily by the underlying market forces of supply and demand - but indirectly so.  This is one area of pricing that has not received enough study by economists to try to figure out the impact of market power on medical care pricing.
